The interaction of all the various Cannabinoid compounds – unabridged – within the Cannabis plant, is known as the ‘Entourage Effect.’
First described in 1998 by Israeli scientists Shimon Ben-Shabat and Raphael Mechoulam, the basic idea of the entourage effect is that cannabinoids within the cannabis plant work together, or possess synergy, and affect the body in a mechanism similar to the body’s own endocannabinoid system. This theory serves as the foundation for a controversial idea within pharmacology community, that in certain cases whole plant extractions serve as better therapeutic agents than individual cannabinoid extractions.
Research on the benefits of THC and CBD, in isolation, have been tested since the late 60ies. THC demonstrates analgesic, anti-emetic, and anti-inflammatory properties. CBD possesses anti-psychotic, anti-seizure, and anti-anxiety properties. Scientific evidence is mounting however that concludes that by isolating these cannabinoids or creating them in a lab, that the resulting effects may limit the potential therapeutic effect. THC alone, when delivered in high enough concentrations, is undesirable. Although rarely requiring medical intervention the side effects of high dosage of THC can be very unpleasant. Current research (see the work of Dr Ethan Russo) is showing that THC and CBD work together. CBD is known to block THC at the CB1 receptor. Therefore demonstrating the entourage effect, the presence of CBD may modulate / decrease the negative effects of THC.
There are no reported overdoses of Marinol. This is pure, lab-produced / synthetic form of THC is not pleasant to take. Initially in the mid-80s, it was thought that Marinol would have the same effect as the cannabis plant as a whole. However, it soon became clear that patients were not responding the same as would be expected by smoking or ingesting whole plant cannabis and it’s contained natural levels of THC. Researchers realised that other compounds, including CBD and various other cannabinoids and terpenes, play a more significant part in the success of the interaction.
The entourage effect theory as described by Wagner and Ulrich-Merzenich, define the four basic mechanisms of whole plant extract synergy as follows: - Ability to affect multiple targets within the body - Ability to improve the absorption of active ingredients - Ability to overcome bacterial defense mechanisms - Ability to minimize adverse side effects. THC and CBD, PLUS the other remaining cannabinoids, rather than compete with one another, work in tandem / synergistically alongside one another, in WHOLE plant cannabis extractions, to provide therapeutic relief for a wide variety of ailments. - Over 100 phytocannabinoids have been identified - The main compounds - a THC–CBD combination - have been shown to provide a larger effect than a summation of those from the compounds separately.
Cannabis History in SA Kevin Bloom of the Daily Maverick – a daily on-line news agent – has published a series of articles that answers hundreds of questions starting from the origin of the use of cannabis through to the 2017 Judgment by Dennis Davis as to the right of a South African to grow and consume cannabis in the privacy of their own home. Kevin wrote a number of articles entitled The Cannabis Chronicles and we recommend that any interested party read these for a more informed perspective on the status quo of Cannabis in South Africa.
On 18 September 2018 a full bench of the high court under Judge Raymond Zondo unanimously legalised the use of Cannabis by adult individuals in private places, as well as the growing of cannabis for private consumption.
